The competition is for the provision of a system for digital home follow-up (DHO), for the purpose of enabling uniform and coherent DHO services for patients/users who receive follow-up, also across service level. The system, including measuring equipment, is delivered as ongoing services.
In the Health Community Ahus and the municipalities, there is an ongoing joint interaction project within digital home follow-up where Ahus and 10 of the municipalities are included. In this project, digital home follow-up is organised in close interaction between the municipal follow-up service, specialist and general practitioner, which promotes the quality of the digital self-treatment plans the patient receives.
